  • Age Well Albany is a fee for service, however most of the costs will be reimbursed to you through Medicare.

    Australian health funds and insurance do not cover the consultation cost of medical specialists, including geriatricians.

    Call us and we will explain what you should expect to pay, depending on your situation.

  • It’s OK to just bring yourself! However, a list of your current prescribed medicines and other medications is helpful. You could also bring any medical letters or documents that you think are relevant.

  • A geriatrician is a specialist doctor trained to manage a broad range of medical conditions including age related problems.

    Problems associated with age include dementia, Parkinson’s Disease, walking problems, frailty and osteoporosis.

    Geriatricians know the importance of treating the whole person and recognise that a person’s priorities and health goals change. This means that a team approach is needed and geriatrician’s collaborate with family and like working within a team of like-minded health worker such and physiotherapists and a patient’s own GP.

  • This is a multidimensional assessment undertaken by a geriatrician to determine the medical, psychosocial and functional capabilities of an older person. This allows a coordinated integrated plan for treatment and long term follow up.

  • We encourage people who support you, such as family, to attend and participate. It’s also helpful if you or your family write down any questions or concerns you want to talk about.

  • Age Well Albany collaborates with external health professionals and care agencies to support you when needed.

    Examples include Aged Care Assessment Team (ACAT), Neurological Council of Western Australia (NCWA), Alzheimer’s Western Australia, Albany Hospital, Home Care Package Providers, Residential Age Care Facilities and a range of local primary care providers.

  • Your GP is incredibly important. For most people, they are the doctor you see most often and who has known you the longest.

    Your geriatrician will communicate, plan and work with your GP to support you through the challenges of aging.

    Your geriatrician will also communicate with other health professionals involved in your care to ensure consistent and co-ordinated treatment and support.

  • Families are often involved and vital in supporting, making plans and important decisions about the care of their older family members.

    A good geriatrician can make a huge difference for the better in a family’s journey into the world of aging and all its related issues and challenges.
